Moving interdisciplinary combined physical and psychological treatment for back pain to a digital format

In our Back To Life service, we normally run monthly, interdisciplinary group based, combined physical and psychological treatment programmes for patients with persistent back pain, living in Birmingham and the West Midlands region. As we can no longer see patients face to face within this service, we are developing an online programme, mixing pre-recorded education videos with facilitated interdisciplinary 'live' online groups sessions with patients, involving education, exercise and relaxation / mindfulness. This will provide an effective way of delivering combined physical and psychological treatment for back pain remotely and we plan to collect outcome data to test its effectiveness on pain disability, self confidence, mood and patients satisfaction. The expectation is that we will be able to provide the same outcomes using a remote service. This allows the service to be more flexible and adaptable, as patients no longer need to travel to up to 9 sessions of treatment and can access treatment in their own home or workplace. More than 50% of patients surveyed have indicated they would be more than happy to receive this treatment online. We know this service significantly improves both physical and psychological health, and, with the fallout of the Covid-19 pandemic likely to impact greatly on the mental health of the nation, we believe this programme will help to address some of these needs

What is needed to sustain the change?

This is a fully commissioned service locally, and, with the support of our IT and communications department, we would be keen to work with other clinical partners to expand this service beyond the boundaries of Birmingham and the West Midlands. Spread would require national commissioning arrangements. Behavioral change to encourage clinicians to use this service needs more work from an educational perspective. This treatment approach is fully endorsed in national clinical guidelines but implementation is currently sparse

The Back to Life service is well esthablished and I know you helped your 1000 patient this year. Well done! .
How do you foresee the digital approach progressing? I've found the shared experience aspect of group typically helps patients.do you think some of the beneficial social interaction will be affected and may affect outcomes?.

Really keen to keep the group effect going as it certainly adds to the therapeutic experience for patients in my opinion. So mixing recorded content with interactive reflective sessions afterwards seems a good step to take. Will be interesting to see how it is received by our patient group and what the uptake is like - will keep you posted
