Self- reporting risk scoring

Rapid identification of high risk /extremely vulnerable patients who should shield during the covid-19 pandemic was a challenge due to the incomplete data held in secondary care regarding multi-morbidities. We identified that patients could rapidly provide updated health information which could inform a more accurate risk  score based upon the BSR risk stratification tool. We formed a ‘risk’ clinical group and held daily virtual huddles via VC to focus on generating the list for NHSE. We utilised a combination of pharmacy prescribing data and using excel formulae generated a provisional score for each patient. We then generated a patient self-reporting digital questionnaire Using www.snartsurvey.co.uk ( IG approved) with question logic and scoring based upon the BSR tool. 2500 + patients responded to the questionnaire. Where there was disagreement in ‘clinician’ and ‘patient’ score , the clinical notes were reviewed. Using a self-reported risk scoring system saved considerable consultant time, and enabled the department to generate a list of extremely vulnerable patients in a timely manner.

- interested in how the survey was shared with patients, if any thoughts or plans for using a survey approach for other aspects of triage, information gathering or care delivery going forward and any advice or tips for others with this method? Thanks!
