stratifying patients at first contact - considering using patient activation measure to assess level of activation and assigning to appropriate care

we are just looking to pilot this but there is some evidence that this will reduce the number of patients needing intervention

What is needed to sustain the change?

continue to look at piloting this so we can get enough data to test are assumptions.

will need GPs  and clinicians to shift their thinking 

training for staff

Thanls laura- do you think staff and patients will be on nboard with the changes and how do we take them with us on this journey?

We have a self management lead who has helped to change the culture around SDM in the last few years and this will help to build on this, we are using our SDM champions to help facilitate this change, putting the patient first, but it will need significant support (and training) to change the approach for some and the self management part of the website will be key!

We are spending some time understanding the PAM and exploring how best to implement this measure... there are complications around its use- more so in regards to the persons health literacy and the time required to ensure it draws out the truth and does not just become another tick box... will keep you posted on our forward movements.

This sounds really interesting Laura.

Apologies if a silly question - by intervention do you mean invasive management, or is that referring to any appointment/interaction with the service rather than self management advice etc?
