We have used telehealth consultations with our patients at the first patient contact, offering video consultations where appropriate. We have used our website signpost and ensure patients have been able to self manage where appropriate .

Increased satisfaction from patients 

Reduced time of some consultations  (not all as some complexity)

 

What is needed to sustain the change?

Enhance the website to further support patients particularly where patients are identified as being 'activated' to self manage.

At NWBH we have used a very similar approach, however we need to further evaluate the use of remote consultations and gain greater clarity regarding which patient groups benefit from this approach. With increased use of telephone and video consultation we have not as yet cut contact times with patients i.e. remote consultations are the same length of time as previous face to face consultation. We took this approach to allow greater time for self management advice at initial consultation

we are doing a piece of work around this - we think a phone call early may reduce further input later down the line but not tested this assumption yet. and important to give good self management advice to support this assumption. working on this.

Hi Laura, I am sure you will share the finding later. Just out of curiosity, are you stratifying patients on the Basis of age, severity and part of body involved. Are you also looking of time scale on when they can come back eg OA Knee- self manage for 6/12 etc. Thanks a lot.

Have you combined these consultations with patient data e.g. home BP monitoring, Blood Glucose readings , wearable tech such as pedometers to promote and monitor interventions?

Similar approach in Northumbria NHS . Amazed how quickly we were able to transition to Video and telephone consultations. many percieved barriers evaporated. major issue at present is availability of hardware -head sets /cameras/ pcs etc to roll out to all.
Have you had any issues with band width / number of phone lines?
would love to hear early evaluation outcomes as only a few studies out there.

Hi Diarmaid, we have trialled a number of platforms and we have a short and long term plan. We are using smart phones in some cases whilst we iron out the issues (as you say webcams etc) we work in single rooms with dedicated phoneline so makes it easier. Bandwidth can be an issue again we are looking at this -what we don't know is the implications of multiple users at the same time! work in progress

We have also been doing purely telehealth consultations in mskcats - we just didn't have the resources to advance to virtual - although that is coming. We have noticed that some body parts and conditions are more difficult to get a clear working diagnosis during a telehealth consult and this may be useful to know if going forward we start all triage with tel or virtual consults? Has any one else found this??
